Application to have a conduct record adjusted
Heavy vehicle owners or operators (HVOOs) may apply to have an event in their conduct record withdrawn or amended, provided certain conditions are met.
Events involving your heavy vehicles impact your conduct review
Under the Conduct Review Policy for Heavy Vehicle Owners and Operators (PDF, 7.8 MB), whenever any of your heavy vehicles are involved in an event that must be taken into consideration in reviewing and assessing your conduct, we enter that event in your HVOO conduct record as soon as we are informed of it.
Please note that it is your responsibility to check your conduct record on a regular basis to stay abreast of any events that may have been entered, and to have any adjustments made, if need be.
How to apply to have your record adjusted
If you wish to apply to have a correction made to an event that has been entered in your record, you must provide the SAAQ with the required information or proof in support of your application and use the form referred to below. You will receive confirmation of any adjustments made in a letter, along with a copy of your revised conduct record.

You must file your application using the Application to Have a Conduct Record Adjusted – Heavy Vehicle Owners, Operators or Drivers (PDF, 137.9 KB) form. Be sure to provide all the required information.
