State of Health and Driver's Licences

In Québec, all drivers must satisfy health requirements, including vision. They are also required to notify the SAAQ of any change that could hinder their ability to drive.

Anyone who has doubts about another person’s ability to safely drive a vehicle can notify the SAAQ. You must be able to provide identifying information, such as the person’s:

  • driver’s licence number;
  • first and last name and date of birth;
  • first and last name and address;
  • first and last name and licence plate number.

The licence plate number on its own is not sufficient.

You can also file a report online through secure email, but you will have to identify yourself.

1 What you should know

Mandatory medical assessments: for your safety and the safety of others

Regardless of the vehicle you drive or your age, driving requires sound physical and mental health, along with good vision.

In addition to the mandatory medical assessments – for which we will send you a letter – you must notify us of any change in your state of health that could hinder your ability to drive within 30 days of that change.

The frequency of the medical assessments or vision tests varies according to:

  • the class of driver's licence
  • your age
  • your state of health (for example, if an illness is reported that can affect your ability to drive) 

If you hold a Class 5 licence (passenger vehicle), a Class 6A, 6B, 6C or 6E licence (motorcycle), a Class 6D licence (moped or scooter) or a Class 8 licence (farm tractor), once you turn 75, you must disclose your state of health using the Self-Declaration of Medical Information (PDF, 145.5 KB) form. You will be sent a copy of the form along with an explanatory letter 45 days before your birthday. Following the analysis of your self-declaration, medical forms may be requested. 

Once you turn 80, and every two years thereafter, you will have to undergo a health assessment . You will be sent the forms to be completed by the appropriate health care professional 45 days before your birthday, along with an explanatory letter.

If you hold a Class 1, 2, or 3 licence (road tractor, bus, straight-body truck), a Class 4A or 4B licence (emergency vehicle or minibus), you are required to provide us with a medical report on your state of health:

  • at ages 45, 55, 60 and 65
  • every 2 years thereafter

To drive a vehicle in the United States that requires a Class 1, 2, 3 or 4B licence, assessments are required:

  • at ages 20, 25, 30, 35, 40 and 45 
  • at ages 48, 51, 54, 55, 57, 60, 63 and 65 
  • every year thereafter

Make your medical appointment as soon as possible

In order to prevent the suspension of your licence or of your right to obtain one, you must make an appointment as soon as possible with your health care professional and send us the Report on a Person's State of Health (PDF, 287.0 KB) before the prescribed deadline.
