Intervention Reports
Carrier enforcement officers may produce two types of reports following an intervention.
Contrôle routier Québec (in French only) performs roadside inspections and facility audits that target heavy vehicle drivers as well as heavy vehicle owners and operators (HVOOs).
Following most of these inspections and audits, two types of reports may be produced:
- A Rapport d’intervention sur route (roadside intervention report) is given to heavy vehicle drivers.
- A Rapport d’intervention en entreprise (facility intervention report) is given to HVOO representatives.
These reports summarize:
- the inspections performed
- the offences committed
- the conclusions of the intervention
- the sources of information that heavy vehicle owners and operators can consult to comply with applicable laws and regulations
Requesting a copy of an intervention report
To receive an additional copy of one or more intervention reports produced within the last two years, authorized HVOO representatives must fill out the form entitled Demande de copies des rapports d’intervention sur route et en entreprise (PDF, 170.1 KB) (in French only).
You are not the HVOO?
Reports can be sent to a person other than the HVOO, provided that the section entitled “Autorisation pour l’envoi des documents à une autre personne” in the form is filled out.
This form can be submitted to CRQ in various ways:
