Your state of health has changed
You are required to report any health problem that could hinder your ability to drive safely, regardless of your age or the class of licence you hold.
You must notify us if your state of health has changed:
- before obtaining a driver's licence or a new class of licence
- at the time of your driver's licence payment or renewal
- at any time, within 30 days of a diagnosis
We will study your declaration and send you a letter informing you of our assessment of the situation.
If your state of health has changed, there are different ways you can inform us:
Online
You can make your medical declaration through your SAAQclic account in the State of Health section.
By mail
You can send your self-declaration form (PDF, 145.5 KB) or the detachable portion of your annual driver’s licence renewal notice to:
