Completing a medical visit report
You can use this online service to complete your medical visit report.
You can provide information about your medical visit on the interactive form, which can then be submitted automatically to the SAAQ.
Before you start
Have on hand:
- your claim number
- personal information that will be used to identify you (last name at birth, first name, date of birth and postal code)
You must keep your claim number and personal information confidential.
Any accident victim who must provide a medical visit report at the request of the SAAQ.
Allow several days for the information you submit to be processed.

To use the SAAQ’s online services, you must enable cookies in your browser.
Please do not use trial versions of software or software under development (alpha, beta, RC).
Online service hours
Monday through Saturday, from 7:30 a.m. to 11:00 p.m.
Sunday, from noon to 11:00 p.m.
Important: Any operation completed after 11:00 p.m. will not be recorded.
