Sending the reports
Once your reports have been completed and signed by the physician or specialized nurse practitioner or the optometrist or ophthalmologist, there are 3 ways you can send them to us :
Online
You can upload your reports to your SAAQclic account.
By mail
You can send your reports to:

In person
You can go to a service outlet to hand in your reports.
Pay attention to the deadline!
It is important that you send us your reports before the deadline indicated in your letter to avoid the suspension of your driver’s licence or your right to obtain one.
However, under certain conditions, you can apply for a deadline extension through SAAQclic .
