Vision test
You have to schedule an appointment with an optometrist who is a member of the Ordre des optométristes du Québec or with an ophthalmologist (websites in French only).
On the day of the test, bring the forms we enclosed with your letter for the optometrist or ophthalmologist to fill out and sign.
Finding an optometrist or an ophthalmologist
We cannot help you find an optometrist or an ophthalmologist, nor can we help you schedule an appointment more quickly. We suggest you go to the website of the Ordre des optométristes du Québec or the Association des médecins ophtalmologistes du Québec (websites in French only) to find one in your region.
Schedule an appointment as soon as possible
It is important that you send us the eye examination report (PDF, 253.2 KB) before the deadline indicated in your letter to avoid the suspension of your driver’s licence or your right to obtain one.
Note that an updated version of the eye examination report form has been in use since April 8, 2026. We wish to reassure you that this will not in any way change your eye examination or affect the SAAQ’s analysis of your fitness to drive.
