The SAAQ offers the reimbursement of psychoeducation services that are covered and given by psychoeducators to accident victims. The SAAQ asks that you claim a reimbursement for psychoeducation services at the end of the authorized block of hours, using the form for that purpose.
This form is available as a dynamic PDF. You may:
- print the form, fill it out by hand, sign it, and send it to the SAAQ using the Envoi de documents et remboursement de frais pour les intervenants externes online service (in French only), or by fax or mail;
OR
- fill out the form onscreen, print it, sign it and send it to the SAAQ using the Envoi de documents et remboursement de frais pour les intervenants externes online service (in French only), or by fax or mail.
