Reports to be filled out by physicians

The SAAQ provides four medical reports for physicians in order to facilitate the processing of claims for accident victims. These reports must be filled out at the appropriate time.

These reports and forms are available as dynamic PDF files. You can:

  • fill out a report onscreen, print it out, sign it and give it to the accident victim. (Should you choose this option, the information you provide regarding the accident victim—first name, last name, claim number, type of report—will be entered automatically on the receipt that comes with the report)

Initial report

The initial report will be reimbursed even if it was not initially requested by the SAAQ.

  • the French and English versions of the Initial Report, the Invoice for an Initial Report and business cards that can be given to people who have been in an accident
  • French versions of a poster for traffic accident victims and a folder about medical reports

Physicians are asked to fill out the Initial Report following the first medical consultation after a traffic accident.  This report comes with an invoice for the physician’s professional fees. Once completed, the report must be given to the accident victim, who will send it to the SAAQ along with the claim for compensation.

Assessment reports, progress reports and impairment reports

The other medical reports serve to provide additional information that allows the SAAQ to understand changes in the accident victim’s medical condition over time.  

Where required, the SAAQ sends the accident victim a preidentified report along with the accompanying receipt or invoice. You must submit a copy of the form to the SAAQ and give the original copy of the form to the accident victim for his or her files.

The reports can also be filled out onscreen. If you fill them out onscreen, you must enter the information used to identify the accident victim in Section 1, and the claim number and communication number in the bottom right-hand corner of the first page. The claim number will be entered automatically on the other pages.

The purpose of this report is to provide an overview of the accident victim’s medical condition.

This report must be filled out when the accident victim’s medical condition does not progress as expected.

After-Effects Report and Receipt or Invoice for a Report (PDF, 355.3 KB)

The purpose of this report is to assess whether the accident victim has been left with any permanent impairment as a result of the injuries sustained in the accident.

Complementary medical information

To adequately assess whether an accident victim has functional limitations or restrictions, the SAAQ may require supplementary medical information.
