      • We will refund you up to $80 for the cost of the detailed medical report issued by your physician.

Once we have received your documents, your application will be analyzed and a written decision will be mailed to you.

If you disagree with the decision, call us for more information. If you are still unsatisfied, you have the right to contest the decision.
