You may request, for yourself or for another person, a copy of the SAAQ compensation file.
You will receive a copy of the file within 30 days following receipt of your request.
You do not have to fill out the form if you are appealing an SAAQ decision before the Administrative Tribunal of Québec. In such a case, you will automatically receive, free of charge, a copy of the documents related to the decision under appeal.
If you wish to submit a request for access to your file under the Act respecting Access to documents held by public bodies and the Protection of personal information, you may submit your request in writing to the person in charge of access to information.
